chore: 同步版本号到 1.9.2、整理 CHANGELOG 并移除误提交的 exe

Browse Source 
- README 版本徽章与飞牛安装包名对齐到 1.9.2
- web/package-lock.json 根版本与 web/package.json 保持一致（原为 1.9.0）
- CHANGELOG 合并整理 1.1.0 起的相近补丁版本并重新编排版本号
- 取消跟踪 Windows 构建产物 havline-agent.exe，并加入 .gitignore
